Registration forms can be completed in person at either of our practice sites, the registration form is also on our website

Patients are also asked to complete medical questionnaire when they register – this can be done in person, or through an online form on our website

We also have an accessible needs information questionnaire for those with a disability, impairment or sensory loss

Details of our practice boundary can be found on our practice website – prospective patients can check whether they fall into our practice boundary with reception if they prefer

How to see a GP

 

Book a routine GP appointment?

 

Patients are encouraged to request a GP appointment using our online system: Patchs. This online system asks patients to answer questions relating to their query to determine whether they need a GP appointment, whether they need investigations first, how urgently they need to be seen or whether their concern would be better dealt with by another primary care team member. For those patients unable to access or complete a Patchs request online reception would complete a form over the phone on their behalf. Relatives can also register for a Patchs account and complete this on their behalf

Book an urgent GP appointment?

 

Patients are encouraged to request an urgent GP appointment using our online system: Patchs as above, reception can complete the Patchs form oner the phone on patient’s behalf where necessary. Urgent queries are specifically flagged as such to the On Call GP

Any patients with life-threatening symptoms e.g. chest pain or difficulty breathing are advised to call 999 rather than request a GP appointment

 

Request a home visit?

 

Patients are encouraged to request a home visit using our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary. We ask that any home visit requests are made before 10am if possible. These requests are reviewed by the GPs who may contact you to ask further information prior to attending. We ask that home visits requests are reserved for those patients who are truly housebound due to illness or disability.

Arranging tests and chronic condition reviews

 

Arrange a review of my chronic disease?

 

Patients will be invited to attend for chronic disease reviews when they are due by the practice. If you think are due or wish to arrange one and have not been invited then please raise your query via Patchs, reception can complete the Patchs form on patient’s behalf where necessary.

Arrange a blood test?

 

Patients are asked to request a blood tests using our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary. If you have been asked to arrange blood tests by a GP/clinician at the practice these can be booked in directly via Patchs or over the phone, we may send an online link to arrange this if you prefer. If you are requesting blood tests for a specific new problem we will ask for further information via Patchs to ensure the appropriate tests are requested. If a clinician outside the practice has requested blood tests we ask for this detail in writing in order to review your request.

Vaccinations

 

Book an appointment for routine vaccinations?

Patients can book in for routine vaccinations when they are due via our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary. 

Book an appointment for travel vaccinations?

Patients are asked to complete a ‘travel form’ with details of their planned trip – this can be done in person via reception or via our online system Patchs. Our nurses will review this information and advise on vaccines needed. Please note we do not offer many travel vaccines and you may be directed to a private service to arrange vaccines as this is not NHS work. Please contact us with your query at least 6 weeks prior to your planned trip.

Contraception

 

Book an appointment to discuss new contraception or for a pill check?

 

Patients can book in to discuss starting contraception with a GP – this can be done via our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary.  If you are already taking the contraceptive pill and need more again please contact us via Patchs – in some cases we can send a questionnaire and do this remotely if you can submit an up to date blood pressure and weight reading. Pill checks can also be arranged in person with the nurse routinely again via Patchs. If you are running short of your pill supply whilst awaiting a review we can often offer a month’s emergency supply of your medication.

Book an appointment for contraception?

 

For a repeat request of your ongoing contraception please contact us online via Patchs, reception can complete the Patchs form on patient’s behalf where necessary. Some contraceptive repeat requests can be dealt with remotely, others will require a nurse appointment. Please make your request in plenty of time to ensure your contraceptive cover is not interrupted.

Arrange to have a coil fitted or removed?

 

Patients wanting a coil should request an appointment with a GP to discuss this – this can be done via our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary.  After assessment we can refer patients to a local surgery for coil fitting or replacement. Patients can also self-refer to the Locala Sexual Health Service for coil fittings or changes https://www.locala.org.uk/services/sexual-health/

Some of our GPs are trained to remove coils – please raise your query via our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary – we can advise you further as necessary.

Arrange to have a contraceptive implant fitted, removed or replaced?

Patients wanting an implant should request an appointment with a GP to discuss this – this can be done via our online system: Patchs, reception can complete the Patchs form on patient’s behalf where necessary.  After assessment we can refer patients to a local surgery for implant fitting or replacement. Book a smear test?

 

If your smear is due you can book in for this us online via Patchs, reception can complete the Patchs form on patient’s behalf where necessary. Smears can only be done where are patient is eligible for one and has been invited – you can check this via Patchs. If you feel you need a smear and haven’t been invited please raise your query via Patchs and we can advise you further.

Book a mammogram?

 

Women are eligible for screening every 3 years from age 51-70 - women are invited for mammograms via the screening service. If you have missed your appointment you can rearrange this by calling by calling 01625 661157. Access support to stop smoking?

Medication queries

 

Order repeat prescriptions?

 

The easiest way to request a repeat prescription is via the NHS app. Link here: https://www.nhs.uk/nhs-app/ Please allow 48 hours for your medication to be ready at your chosen chemist

You can also request your repeat medication via Patchs or via the practice email account

Order a prescription for something that is NOT on repeat?

Please make any requests of this nature online via Patchs or ask reception to do this on your behalf – please include details of what medication you would like any why. A GP will review your request – they may ask further questions or request a GP appointment to discuss things further.

Book a medication review?

 

Patients are usually invited for their medication review by the practice when they are due. If you have a query or wish to arrange a medication review and have not been invited please complete an online form via Patchs or ask reception to do this on your behalf – we can advise on a medication query or whether your medication review is due. We will review your records to ensure you are invited in for monitoring blood tests/BP check where appropriate prior to your medication review appointment. Medication review appointments are carried out by our pharmacy teams. We have a Clinical Pharmacist working within the practice and we work with Pharmacists across our Primary Care Network – please be aware of this when making an appointment

Medical forms/letters & private medical exams

 

Arrange to have a form completed (e.g. bus pass form, passport application etc)?

Please leave a copy of such forms with reception or you can email them securely to gmicb-sto.p88005-admin@nhs.net

We have an employment support health coach working with the practice who helps to support patients in getting back to work. We will either offer a fit note, arrange an appointment with our Health Coach or offer a GP appointment depending on the nature of your concern.
