Non-urgent advice: WorkWell Service – Viaduct Care

WorkWell provides support for people whose health is affecting their ability to work, or who need help to return to work.

If you are struggling to stay in employment, are off sick, or finding that a health condition is making work difficult, our team can offer practical, personalised support.

We can help with:

· Managing health conditions in the workplace

· Building confidence to return to work

· Exploring training, volunteering or employment options

· Advice around workplace adjustments

· Linking you with local services for wider support

WorkWell takes a holistic approach, focusing on your strengths and what matters to you.

Who can access the service?

· Aged 18+

· Registered with a Stockport GP

· Experiencing health-related barriers to work

Social Prescribing offers non-medical support to help improve your health and wellbeing.

If things like loneliness, housing issues, financial worries, low mood, managing a long-term condition, or making lifestyle changes are affecting you, our team can help. We take time to understand what matters to you and connect you with local services and community support.

Who can access the service?

· Aged 18+

· Registered with a Stockport GP

· Happy to consent to referral

Appointments can take place by phone, at the practice, in the community or at home where appropriate.
