Request access to my medical records?
All patients can now have prospective access to their online records through the NHS App or patient access services.
- To request online patient access please complete a form provided by reception
- Your request will be reviewed by a GP and we will advise you of the outcome
- You will be able to review your record via the NHS app or patient access app once granted. You can also order repeat prescriptions here.

If you require specific information in your medical records which you cannot see online, or are requesting access to records on behalf of someone else please see below:
- Please complete a Subject Access Request form detailing the information you need and hand this in to reception.
- Your request will be reviewed by a GP and we will advise you of the outcome
- You will be able to review your record via the NHS app or patient access app once granted

- Your request will be reviewed by a GP – your records can then be viewed via the NHS app.

Register for proxy access to view the records of my child / support someone I care for?
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. This can be set up for you if you are both registered with us.
