Join the surgery
Welcome to the Reddish Family Practice
We are always happy to welcome new patients to our practice and our aim is to provide high quality care for all of our patients. The surgery benefits from 2 locations at each side of Reddish with a range of clinics at both.
We have a friendly team of 4 GP Partners and 5 Salaried GPs, and a nursing and support team. The practice provides: onsite phlebotomy, healthchecks, clinics for mother and baby, child vaccinations, Flu and covid vaccinations, travel vaccinations, minor surgery and a range of support services with our physio, care coordinators and pharmacy teams.
We are a Veteran Friendly practice, and have acheived a bronze award in Pride in Practice, and are continuously reviewing feedback and procedures in order to improve on the service we offer to our patients with the help of our Patient Participation Group.
Catchment Area
Please check the practice boundary map to check that you live within the practice catchment area. We cannot accept patients who live outside the area.
You do not need ID, an NHS number or proof of address to register.
We may ask for supporting documentation as it can:
- help the surgery find your medical records or transfer them from your current GP
- confirm that you live in the surgery's area (or "practice boundary") if they do not accept patients from outside this area
If you do not have a permanent address you can still register using a temporary address or the address of the GP surgery.

Register with us
From October 2024 all practices are required to use the PRF1 form for patient registrations. The easiest way to do this is online using the link below:
This will guide you through some medical questions to support the surgery to care for you. Once we have received your application we will process your request within 5-10 working days.
If you prefer you can print off a copy of the form below to hand into our reception team:
Accoutable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
